What should I bring with my child to the appointment?

Depending on the condition that your child has been referred for, the paediatrician may require more information.

  • If you have any reports or letters from other health providers (eg GP, Speech therapy, Physiotherapy, Audiology hearing test results, etc) or others involved in the care of the child (eg daycare, kinder, school etc) please bring these with you.
  • Maternal Child Health Record (blue book or green folder)
  • Some parents make notes about how the condition affects the child and these are also very useful.
  • Sometimes a list of questions that you want to discuss with the paediatrician can help at the time of the appointment.

With all appointments, please bring your Medicare card and any Health Care / Pension cards that you hold with you.
